Contract Type: Contract

Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, with rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop and manage daily patient lists for the palliative care team
Conduct thorough patient chart reviews and assess needs
Facilitate goals of care discussions with patients and families
Provide education on disease progression, advance directives, and POLSTs
Lead family meetings and offer end-of-life support
Attend daily rounds to coordinate multidisciplinary care

Qualifications

2-5 years of recent palliative or hospice nursing experience
ICU or oncology experience a plus
Strong acute care, clinical thinking, and communication skills
Flexibility and excellent organizational abilities
Current BLS certification (ELNEC preferred)

Knowledge & Skills

Expertise in palliative and end-of-life care
Proficiency with Cerner EMR
Ability to conduct comprehensive patient assessments
Skilled in facilitating sensitive conversations
Strong interdisciplinary collaboration skills
