Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Conduct comprehensive psychosocial assessments for patients
Develop and implement care coordination plans
Provide crisis intervention and 5150 evaluations as needed
Collaborate with interdisciplinary team to ensure patient safety and well-being
Advocate for patient resources and discharge planning

Qualifications

Active LCSW license in California
Current BLS certification
5150 certification in California (completed training and passed exam)
Prior experience with Dignity Health or CommonSpirit facilities
2 supervisory references required (1 within 12 months, 1 within 3 years)

Knowledge & Skills

Crisis intervention and de-escalation techniques
Knowledge of 5150 holds and involuntary evaluation procedures
Strong communication and collaboration skills
Compassionate patient-centered approach
