Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, rotating weekends

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive evaluations and develop individualized treatment plans for home health patients
Provide skilled physical therapy interventions including therapeutic exercises, manual therapy, and modalities
Perform start of care, recertifications, and evaluations as needed
Educate patients and caregivers on home exercise programs and fall prevention
Collaborate with interdisciplinary team to ensure coordinated care
Document patient progress accurately and timely in electronic health records

Qualifications

Active Physical Therapist license in Kentucky or compact privilege
BLS certification required
Minimum 1 year of home health experience preferred, but new grads are welcome to apply
Must have reliable transportation and valid driver's license

Education

Graduate of an accredited Physical Therapy program (DPT or MPT)

Knowledge & Skills

Strong clinical reasoning and assessment skills
Ability to work independently in a home health setting
Excellent communication and patient education abilities
Proficient with electronic documentation and telehealth if needed
