Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive physical therapy assessments and develop personalized treatment plans
Provide skilled therapy services to patients in their homes, focusing on orthopedic and geriatric conditions
Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
Educate patients and families on exercises and strategies to improve mobility and independence
Document patient progress accurately in HCHB (HomeCare HomeBase) EMR system

Qualifications

Active Physical Therapy license in California (or ability to obtain)
Current BLS certification from the American Heart Association
Minimum 2 years of experience in home health (travelers with less considered if they have home health experience)
Experience with HCHB EMR preferred

Knowledge & Skills

Strong knowledge of home health regulations and OASIS documentation
Excellent clinical assessment and problem-solving skills
Ability to work independently and manage a daily caseload of 5 visits
Compassionate communication and patient education skills
