Roles & Responsibilities

Provide high-quality physical therapy services to residents in a skilled nursing facility
Develop and implement individualized treatment plans to improve mobility and function
Collaborate with interdisciplinary team members to ensure comprehensive care
Document patient progress and adjust treatments as needed
Educate patients and families on therapeutic exercises and home programs
Contribute to a positive and healing environment for residents

Qualifications

Active Physical Therapist (PT) license in Illinois (or compact license eligible)
Doctorate in Physical Therapy (DPT) required
New graduates are welcome to apply
Experience in skilled nursing or geriatrics is a plus, but not required

Education

Doctorate in Physical Therapy (DPT) from an accredited program

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent communication and interpersonal abilities
Ability to work independently and as part of a team
Proficient in electronic medical records (EMR) documentation
Passion for helping older adults achieve their best level of function
