Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to home health patients within a 60-mile radius of Valley City
Develop and implement individualized treatment plans for patients recovering from surgeries, falls, and other conditions
Collaborate with interdisciplinary team members to ensure coordinated patient care
Document patient progress accurately using HomeCare/HomeBase EMR system
Educate patients and families on home exercise programs and safety strategies

Qualifications

Active North Dakota Physical Therapy license
Current BLS certification
Minimum 1 year of recent physical therapy experience
Previous home health experience preferred
Valid driver's license and auto insurance required

Education

Graduate of an accredited physical therapy program

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Proficient in HomeCare/HomeBase or similar EMR systems
Excellent communication and patient education abilities
Ability to work independently with minimal supervision
Comfortable traveling to patient homes within a 60-mile radius
