Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct thorough patient evaluations and develop personalized treatment plans
Perform start of care, recertifications, and daily visits in a home health setting
Collaborate with a supportive clinical manager and educator available 24/7
Document patient progress and adjust care plans as needed
Provide education to patients and families on therapeutic exercises and safety

Qualifications

Active Physical Therapist license in Kentucky
BLS certification required
Prefer experience, but new grads with strong clinical skills are welcomed
Must be comfortable with home health visits and driving to patient locations

Knowledge & Skills

Expertise in managing CHF, ortho, and pneumonia patients
Proficient in start of care, recertifications, and evaluations
Strong communication and time management skills
Ability to work independently in a home health setting
