Roles & Responsibilities

Provide compassionate physical therapy services to residents in a skilled nursing facility
Develop and implement individualized treatment plans to improve mobility and function
Collaborate with an interdisciplinary team to ensure holistic care
Monitor and document patient progress, adjusting treatments as needed
Educate patients and families on exercise programs and safety strategies

Qualifications

Valid Physical Therapist (PT) license in Massachusetts
New graduates are welcome to apply
Experience in SNF or long-term care settings is a plus
Ability to work independently and as part of a team
Strong communication and interpersonal skills

Education

Doctor of Physical Therapy (DPT) degree from an accredited program

Knowledge & Skills

Expertise in physical therapy evaluation and treatment
Knowledge of SNF regulations and documentation standards
Proficiency with electronic medical records (EMR)
Excellent problem-solving and critical-thinking skills
Ability to adapt to changing patient needs
