Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with rotating Saturdays once a month

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in a home health setting
Develop and implement individualized treatment plans based on physician's orders
Instruct patients and caregivers on therapeutic exercises and use of equipment
Monitor and document patient progress, adjusting treatment as needed
Collaborate with the healthcare team to ensure comprehensive care
Educate families on home exercise programs and safety measures

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute care, home health, or rehab setting
Current BLS certification required
Valid Physical Therapist license in California
Excellent communication and organizational skills
Ability to work independently and manage a caseload across Yolo County

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Knowledge of home health regulations and Medicare conditions of participation
Proficient in documenting patient care and progress
Ability to educate and motivate patients and families
