Location: Red Bluff, CA

End Date: 09-15-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, Day shift (8:00 AM - 5:00 PM), no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive home health assessments and develop individualized treatment plans
Perform start-of-care visits for orthopedic and geriatric patients
Manage patients with multiple comorbidities including CHF and COPD
Collaborate with interdisciplinary team to ensure coordinated care
Document patient progress and maintain accurate electronic health records in HCHB
Educate patients and families on treatment plans and home exercise programs

Qualifications

Active Physical Therapist license in California
BLS certification required
Minimum 1 year of Home Health experience preferred
New graduates with strong Home Health clinicals or experience considered

Knowledge & Skills

Expertise in orthopedic and geriatric care
Experience with CHF and COPD management
Proficiency in HCHB electronic medical record system
Strong assessment and clinical documentation skills
Excellent communication and patient education abilities
