Roles & Responsibilities
- Assist in carrying out treatment plans developed by the supervising Physical Therapist to help patients regain mobility
- Provide therapeutic exercises, gait training, balance training, and mobility activities tailored to each resident
- Monitor patient progress and communicate notable changes to the supervising therapist
- Educate patients and caregivers on exercises, mobility techniques, and safety practices to support independence
- Maintain accurate and timely documentation of therapy sessions to ensure continuity of care
- Collaborate with interdisciplinary team members to support rehabilitation goals and holistic patient recovery
Qualifications
- Active California Physical Therapist Assistant (PTA) license
- Associate degree from an accredited PTA program
- Previous experience in SNF, rehabilitation, or geriatric care preferred
- Strong communication and patient care skills
- Ability to work effectively within a multidisciplinary healthcare team
- Available for full-time work in a Skilled Nursing Facility setting
Education
- Associate degree from an accredited Physical Therapist Assistant (PTA) program
Knowledge & Skills
- Knowledge of therapeutic exercise techniques and mobility training
- Strong patient monitoring and documentation skills
- Excellent teamwork and communication abilities
- Ability to educate patients and caregivers on safe mobility and exercises
- Comfort working under the supervision of a Physical Therapist and following care plans
