Roles & Responsibilities

• Provide compassionate, skilled occupational therapy services to skilled nursing facility residents
• Collaborate with interdisciplinary team members to create meaningful, patient-centered care plans
• Deliver therapeutic interventions to improve daily living skills, mobility, and independence
• Document progress, outcomes, and discharge planning in a timely and compliant manner
• Communicate clearly with residents, families, and staff to set realistic goals and celebrate gains
• Adapt treatment approaches when floating between Middletown and Centerville to meet resident needs

Qualifications

• Active COTA certification (Certified Occupational Therapy Assistant) or eligibility to obtain state certification
• Graduate of an accredited OTA program; associate degree typically required
• Comfortable working in a Skilled Nursing Facility (SNF) setting
• Open to clinicians at all experience levels including new grads
• Ability to float to a sister location in Centerville as needed
• Strong documentation skills and ability to work collaboratively on a therapy team

Education

• Graduate of an accredited Occupational Therapy Assistant (OTA) program (Associate degree preferred)

Knowledge & Skills

• Foundational occupational therapy principles and adaptive equipment knowledge
• Skilled in resident assessment, treatment planning, and progress documentation
• Familiarity with SNF workflows, regulatory documentation, and rehab-focused goals
• Good communication and interpersonal skills for resident and family interactions
• Ability to prioritize caseload and manage time efficiently during day shifts
