Roles & Responsibilities
- Provide compassionate, direct patient care to residents according to care plans
- Administer medications and treatments safely and accurately
- Assess and monitor patient condition; report changes promptly to the care team
- Document assessments, interventions, and patient responses in the medical record
- Collaborate with nurses, clinicians, and support staff to deliver coordinated care
- Support patients and families with education, comfort, and clear communication
- Adhere to infection control, safety, and facility policies at all times
Qualifications
- Current, active LPN license (must be valid in New Mexico)
- Basic Life Support (BLS) / CPR certification required or willing to obtain
- Prior LPN experience in long-term care or similar setting preferred
- Ability to work per diem schedule, including occasional weekends or holidays
- Strong communication skills and a patient-centered attitude
Education
- Graduate of an accredited Practical Nursing program (LPN/LVN diploma)
Knowledge & Skills
- Proficient in medication administration and documentation
- Skilled in taking and interpreting vital signs and patient assessments
- Familiarity with wound care and basic clinical procedures
- Comfortable with electronic health records and accurate charting
- Excellent interpersonal skills and teamwork
- Solid time management and prioritization in a fast-paced environment
