Roles & Responsibilities
- Provide compassionate direct nursing care to residents according to individualized care plans
- Administer medications and treatments accurately and document thoroughly
- Perform skilled assessments and promptly communicate changes in condition to the care team
- Collaborate with interdisciplinary team members to support resident goals and outcomes
- Support admissions, transfers, and discharge processes with professionalism
- Maintain regulatory and facility documentation standards with attention to detail
- Coach and mentor CNAs and assistive staff to promote high-quality resident care
Qualifications
- Valid LPN license in New Mexico (or compact/ability to obtain) required
- Current BLS (Basic Life Support) certification required
- Experience in long-term care, skilled nursing, or post-acute settings preferred
- Travel LPN experience or flexibility to adapt quickly to facility protocols a plus
- Strong communication skills and ability to work independently on night shift
Education
- Graduate of an accredited Practical Nursing program (LPN diploma or equivalent)
Knowledge & Skills
- Skilled in medication administration and documentation
- Proficient in resident assessment and care planning
- Familiarity with electronic medical records and charting
- Ability to manage acute changes and escalate appropriately
- Infection control and safety practices
- Time management and prioritization in a fast-paced clinical setting
- Team collaboration and clear clinical communication
- Compassionate resident-centered care approach
