Roles & Responsibilities

• Provide compassionate direct nursing care to residents according to individualized care plans
• Administer medications and treatments accurately and document thoroughly
• Perform skilled assessments and promptly communicate changes in condition to the care team
• Collaborate with interdisciplinary team members to support resident goals and outcomes
• Support admissions, transfers, and discharge processes with professionalism
• Maintain regulatory and facility documentation standards with attention to detail
• Coach and mentor CNAs and assistive staff to promote high-quality resident care

Qualifications

• Valid LPN license in New Mexico (or compact/ability to obtain) required
• Current BLS (Basic Life Support) certification required
• Experience in long-term care, skilled nursing, or post-acute settings preferred
• Travel LPN experience or flexibility to adapt quickly to facility protocols a plus
• Strong communication skills and ability to work independently on night shift

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Skilled in medication administration and documentation
• Proficient in resident assessment and care planning
• Familiarity with electronic medical records and charting
• Ability to manage acute changes and escalate appropriately
• Infection control and safety practices
• Time management and prioritization in a fast-paced clinical setting
• Team collaboration and clear clinical communication
• Compassionate resident-centered care approach
