Roles & Responsibilities

• Assist patients with activities of daily living — bathing, dressing, grooming and toileting with dignity and compassion
• Support mobility and transfers, using safe body mechanics and approved equipment
• Monitor and record vital signs, intake/output and changes in condition accurately
• Provide attentive, personalized care for rehab patients working toward recovery goals
• Communicate effectively with nurses, therapists and the care team to ensure coordinated care
• Maintain a clean, safe environment and adhere to infection control protocols
• Document care in the patient record promptly and clearly to support continuity of care

Qualifications

• Current CNA certification (T-CNA/CNA) in good standing — required
• Comfortable working scheduled day shifts: Mon/Tue/Wed, 6:00 AM - 6:30 PM
• CPR/BLS certification preferred or willing to obtain
• Ability to pass background check, drug screen and required health screenings/vaccinations
• Compassionate patient-care approach, strong communication and teamwork skills
• Previous experience in a rehab or long-term care setting is a plus

Education

• High school diploma or equivalent
• Completed state-approved CNA program and active CNA certificate

Knowledge & Skills

• Skilled at assisting with ADLs and mobility support
• Knowledgeable in basic vital sign measurement and documentation
• Familiarity with infection control best practices
• Good communication and interpersonal skills with patients and families
• Comfortable using electronic health record systems for charting
• Ability to follow care plans and work collaboratively with interdisciplinary teams
