Roles & Responsibilities
- Assist patients with activities of daily living — bathing, dressing, grooming and toileting with dignity and compassion
- Support mobility and transfers, using safe body mechanics and approved equipment
- Monitor and record vital signs, intake/output and changes in condition accurately
- Provide attentive, personalized care for rehab patients working toward recovery goals
- Communicate effectively with nurses, therapists and the care team to ensure coordinated care
- Maintain a clean, safe environment and adhere to infection control protocols
- Document care in the patient record promptly and clearly to support continuity of care
Qualifications
- Current CNA certification (T-CNA/CNA) in good standing — required
- Comfortable working scheduled day shifts: Mon/Tue/Wed, 6:00 AM - 6:30 PM
- CPR/BLS certification preferred or willing to obtain
- Ability to pass background check, drug screen and required health screenings/vaccinations
- Compassionate patient-care approach, strong communication and teamwork skills
- Previous experience in a rehab or long-term care setting is a plus
Education
- High school diploma or equivalent
- Completed state-approved CNA program and active CNA certificate
Knowledge & Skills
- Skilled at assisting with ADLs and mobility support
- Knowledgeable in basic vital sign measurement and documentation
- Familiarity with infection control best practices
- Good communication and interpersonal skills with patients and families
- Comfortable using electronic health record systems for charting
- Ability to follow care plans and work collaboratively with interdisciplinary teams
