Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and follow established care plans accurately and safely
• Monitor patient condition, document findings, and communicate changes to the care team
• Support admissions, transfers, and discharges with professionalism and attention to detail
• Assist with wound care, IV support per facility policy, and routine clinical procedures
• Mentor and support nursing assistants and collaborate closely with RNs and interdisciplinary staff
• Contribute to a positive resident experience by delivering respectful, patient-centered care

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing or eligible to obtain state licensure
• Basic Life Support (BLS/CPR) certification or willingness to obtain upon hire
• Clinical experience in long-term care, rehab, or related settings preferred
• Strong clinical assessment, medication administration, and documentation skills
• Reliable, professional, and a team player with strong communication skills
• Immunizations and health screenings as required by facility policy

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program or equivalent vocational training

Knowledge & Skills

• Solid knowledge of LPN scope of practice and long-term care protocols
• Medication administration and charting accuracy
• Patient assessment and recognition of changes in condition
• Infection control, wound care basics, and safe patient handling
• Familiarity with electronic medical records and basic computer skills
• Strong interpersonal skills and the ability to work well in a multidisciplinary team
