Roles & Responsibilities

• Provide compassionate bedside nursing and reliable care for patients in a rehab/acute setting
• Administer medications and treatments safely and accurately according to policies
• Perform focused assessments and communicate changes in patient status to the care team
• Document care clearly in the medical record to ensure continuity and quality
• Assist with activities of daily living while promoting patient independence and dignity
• Collaborate with RNs, therapists and physicians to support individualized care plans
• Maintain infection control and safety standards to protect patients and staff

Qualifications

• Active LPN license (Licensed Practical Nurse) required — must be valid for New Mexico or be able to obtain prior to start
• Current BLS/CPR certification required
• Previous experience in rehabilitation, long-term care, or med-surg settings preferred
• Ability to pass background check, drug screen and required immunizations
• Strong communication skills and ability to work effectively overnight with a multidisciplinary team

Education

• Graduate of an accredited LPN / Practical Nursing program (diploma or certificate)

Knowledge & Skills

• Solid medication administration and documentation skills
• Comfortable with patient assessments, wound care basics and mobility assistance
• Good time management and prioritization during busy night shifts
• Clear verbal and written communication with patients, families and clinical teams
• Familiarity with electronic medical records and basic charting practices
