Roles & Responsibilities

• Provide compassionate, high-quality nursing care to patients in the Uptown Rehab/Health area
• Administer medications and treatments safely and document all care accurately
• Perform thorough patient assessments and monitor changes in condition
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Educate patients and families on care plans, medications and post-discharge needs
• Maintain accurate, timely electronic charting and handoffs to ensure continuity of care
• Promote a safe environment by following infection control and facility policies

Qualifications

• Active, unrestricted Registered Nurse license (RN) — state licensure appropriate for assignment
• Current BLS (Basic Life Support) certification required; additional certifications encouraged
• Clinical experience in rehab, med-surg or related area is a plus
• Strong communication skills and the ability to work collaboratively with interdisciplinary teams
• Professional, reliable, and patient-centered approach to care

Education

• Graduate of an accredited nursing program (ADN or BSN preferred for RN)
• LPN diploma or certificate (if transitioning roles) — RN licensure required for this posting

Knowledge & Skills

• Skilled in patient assessment and vital sign monitoring
• Medication administration and accurate documentation
• Familiarity with rehab patient needs and care planning
• Comfortable using electronic health record systems
• Ability to prioritize, triage and manage a varied patient caseload
• Solid infection control practices and safety awareness
