Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents and patients
• Administer medications and treatments safely and accurately according to orders
• Perform routine assessments, monitor vital signs, and report changes promptly
• Document care clearly and timely in the facility electronic health record
• Support patients with activities of daily living while promoting independence
• Collaborate with RNs, physicians, and the interdisciplinary team to coordinate care
• Adhere to infection control, safety standards, and facility policies at all times
• Participate in care planning and contribute meaningful observations to team discussions

Qualifications

• Current and active Licensed Practical Nurse (LPN) license in the state of practice
• Minimum clinical experience caring for adults or geriatric populations preferred
• Basic Life Support (BLS) certification or willingness to obtain upon hire
• Strong communication skills and ability to work effectively on a team
• Valid CPR/BLS and any state-required certifications up to date

Education

• Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and basic pharmacology knowledge
• Patient assessment, wound care basics, and vital sign monitoring
• Electronic charting and accurate documentation practices
• Infection prevention and standard precautions
• Time management and prioritization in a fast-paced environment
• Empathy, clear communication, and strong teamwork skills
