Certified Nursing Assistant - Skilled Nursing & Rehab
Location: Wausau, WI
Shift Type: Rotating PM/NOC shifts (mix of 8- and 12-hour): 2:00 PM - 10:00 PM (8-hour) and 2:00 PM - 2:00 AM (12-hour); rotating schedule with every-other-weekend and every-other-holiday required
Hours per Week: 48
Candidate Pay Rate: $45.12 - $31.58 per hour

Roles & Responsibilities
- Provide compassionate hands-on resident care including feeding, bathing, grooming, and mobility support
- Assist with ambulation, transfers and positioning using gait belts and mechanical lifts to keep residents safe
- Follow individualized care plans and deliver care aligned with each resident’s goals and preferences
- Promptly respond to call lights and resident needs with empathy and professionalism
- Document ADLs, intake/output and restorative care accurately in the electronic charting system before end of shift
- Observe, report and communicate changes in resident physical, emotional or cognitive status to licensed nursing staff
- Support dining service and mealtime assistance, including set-up and feeding help when needed
- Follow infection control protocols and maintain a clean, safe, organized resident environment
- Participate in onboarding and training of new staff and contribute to a positive team culture
- Support emergency procedures and respond appropriately to codes and security alerts
Qualifications
- Active certification as a Certified Nursing Assistant on the Wisconsin Nurse Aide Registry (required)
- Active BLS certification at time of submission (required)
- Minimum 2+ years of CNA experience (skilled nursing, rehab or long-term care experience preferred)
- Ability to lift 50+ lbs and assist residents safely with mobility and transfers
- Comfortable and competent with electronic medical records and basic computer tasks
- Willingness to float to different units (long-term care, rehab, dementia, ventilator unit) as needed
- Available for rotating PM shifts, every-other-weekend and every-other-holiday coverage
Knowledge & Skills
- Care planning and resident-centered care
- Resident feeding, mobility support and ADL assistance
- Infection control and safety protocols (PPE, hand hygiene)
- Effective communication with residents, families and interdisciplinary teams
- Use of gait belts, sit-to-stand and full-body lifts
- Electronic medical records/charting and accurate documentation
- Dementia care awareness and end-of-life/hospice support experience
