Roles & Responsibilities

• Provide compassionate direct patient care to residents in the Bear Canyon rehabilitation setting
• Administer medications and treatments accurately and safely
• Monitor patient status and promptly report changes to the care team
• Document assessments, interventions and patient progress in the medical record
• Collaborate with nurses, therapists and physicians to deliver coordinated care
• Educate patients and families on care plans and post-discharge needs

Qualifications

• Active Licensed Practical Nurse (LPN) license (New Mexico license preferred or eligible)
• Current Basic Life Support (BLS/CPR) certification or willingness to obtain
• Prior experience in long-term care, rehab or similar settings is a plus but dedicated new practitioners are welcome
• Strong patient assessment and medication administration skills
• Ability to work independently on night shift while communicating effectively with the team

Education

• LPN diploma or certificate from an accredited nursing program; high school diploma or equivalent

Knowledge & Skills

• Solid knowledge of medication administration and documentation standards
• Skilled in wound care, IV/line awareness and common rehab nursing tasks
• Familiarity with infection control practices and safety protocols
• Comfortable using electronic medical records and charting accurately
• Excellent communication and teamwork skills with patients and interdisciplinary staff
