Roles & Responsibilities

• Assess hospice patients in homes and facilities and deliver compassionate, patient-centered nursing care
• Manage an assigned caseload of roughly 12–14 patients and prioritize 4 visits per day or 2 Start of Care visits per day
• Perform and document nursing visits and assessments accurately in Homecare Homebase (HCHB)
• Coordinate care with interdisciplinary team members, families, and community resources to support comfort-focused goals
• Initiate and complete Start of Care (SOC) visits and related documentation in a timely manner
• Communicate changes in patient condition and ensure appropriate follow-up and escalation when needed
• Support patients and families through teaching, symptom management, and emotional support

Qualifications

• Active RN (Registered Nurse) license in California
• BLS (AHA) certification required
• Minimum 1 year of hospice nursing experience required; 2 years of nursing experience noted/preferred
• Comfortable with home health/hospice case management and conducting home visits
• Proficient with Homecare Homebase (HCHB) charting or willing to complete training
• Able to work rotating weekends and assigned holidays as outlined
• Candidates must not have been directly employed by CommonSpirit (including CHI or Dignity) in the past year; PRN employment through outside firms is acceptable
• Willingness to complete department-specific orientation

Education

• Active Registered Nurse (RN) license
• BSN preferred but not required
• BLS (AHA) certification required

Knowledge & Skills

• Strong clinical assessment and symptom management skills for hospice populations
• Experience with Start of Care (SOC) processes and caseload management
• Accurate charting and documentation skills in HCHB (Homecare Homebase)
• Excellent communication and family-support skills for end-of-life care
• Time management and prioritization for multiple daily visits
• Ability to collaborate with interdisciplinary teams and community resources
