Shift Type: Rotating day/evening shifts (primarily weekdays with rotating weekends about every 4 weeks), 12:00 PM - 8:30 PM (every other week pattern noted), 8-hour shifts, approximately 40 hours per week, holiday coverage as assigned (minor holidays and 2 major holidays required)

Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Assess hospice patients in homes and facilities and deliver compassionate, patient-centered nursing care
- Manage an assigned caseload of roughly 12–14 patients and prioritize 4 visits per day or 2 Start of Care visits per day
- Perform and document nursing visits and assessments accurately in Homecare Homebase (HCHB)
- Coordinate care with interdisciplinary team members, families, and community resources to support comfort-focused goals
- Initiate and complete Start of Care (SOC) visits and related documentation in a timely manner
- Communicate changes in patient condition and ensure appropriate follow-up and escalation when needed
- Support patients and families through teaching, symptom management, and emotional support
Qualifications
- Active RN (Registered Nurse) license in California
- BLS (AHA) certification required
- Minimum 1 year of hospice nursing experience required; 2 years of nursing experience noted/preferred
- Comfortable with home health/hospice case management and conducting home visits
- Proficient with Homecare Homebase (HCHB) charting or willing to complete training
- Able to work rotating weekends and assigned holidays as outlined
- Candidates must not have been directly employed by CommonSpirit (including CHI or Dignity) in the past year; PRN employment through outside firms is acceptable
- Willingness to complete department-specific orientation
Education
- Active Registered Nurse (RN) license
- BSN preferred but not required
- BLS (AHA) certification required
Knowledge & Skills
- Strong clinical assessment and symptom management skills for hospice populations
- Experience with Start of Care (SOC) processes and caseload management
- Accurate charting and documentation skills in HCHB (Homecare Homebase)
- Excellent communication and family-support skills for end-of-life care
- Time management and prioritization for multiple daily visits
- Ability to collaborate with interdisciplinary teams and community resources
