Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Evaluate and treat skilled nursing facility patients to improve mobility, strength, and function
- Develop and implement individualized treatment plans based on patient goals and clinical findings
- Complete timely, facility-standard documentation in Net Health
- Collaborate closely with nursing and the interdisciplinary care team to coordinate patient-centered care
- Manage caseload and prioritize time effectively to meet patient and facility needs
- Float to a sister facility within walking distance when coverage is required
Qualifications
- Active CA PT license required
- SNF experience preferred; mentorship available for clinicians with 0–1 years of experience
- Strong documentation skills (experience with Net Health is a plus)
- Excellent time-management and organizational skills
- Flexible, collaborative mindset and willingness to float as needed
- Ability to work clinically with a diverse SNF patient population
