Roles & Responsibilities

• Provide high-quality physical therapy evaluations and hands-on treatment in patients' homes
• Complete accurate OASIS documentation and charting in Cerner EMR to meet regulatory standards
• Develop individualized treatment plans addressing ortho, neuro, joint and spinal injury diagnoses
• Coordinate care with rehab aides, OT, SLP, OTA/PTA and interdisciplinary team members
• Educate patients and caregivers to support safe home mobility, fall prevention and functional independence
• Manage caseload within an assigned geographic region and prioritize visits for patient safety
• Maintain professional attire and represent the organization in patient homes respectfully
• Participate in 1.5–2 weeks of unit-specific orientation and ongoing clinical development

Qualifications

• Active Physical Therapist license (state licensure required)
• Minimum 2 years of clinical experience (job notes indicate 2 years required)
• At least 1 year experience working with PT/OT/SLP patients (home health experience preferred)
• BLS certification required
• Experience with OASIS documentation (explicit requirement)
• Proficiency with Cerner EMR or similar electronic charting systems
• Reliable transportation and ability to cover an assigned geographical region

Knowledge & Skills

• Strong working knowledge of home health practice and post-acute care standards
• Skilled in orthopedic and neurological rehabilitation interventions
• Competent in performing thorough patient evaluations and writing clear treatment plans
• Excellent documentation skills — OASIS and Cerner experience highly valued
• Good communication and caregiver education skills for home-based care
• Ability to collaborate with Rehab Aides, OTA/PTA and the interdisciplinary team
• Time management and route planning for efficient home visit scheduling
