Roles & Responsibilities
- Implement occupational therapy treatment plans to help residents regain functional independence
- Work closely with the supervising OT and interdisciplinary team to coordinate patient-centered care
- Complete timely, accurate documentation in Net Health according to facility standards
- Ensure compliance with Medicare guidelines, facility policies, and regulatory requirements
- Deliver compassionate care for short-term and long-term residents, adapting plans as needs change
- Communicate progress, barriers, and discharge planning details with team members and families
Qualifications
- Active OTA (Certified Occupational Therapy Assistant) license required
- Skilled Nursing Facility (SNF) experience preferred; mentorship available for clinicians with 0-1 year experience
- Strong documentation skills and familiarity with Net Health (or quick willingness to learn)
- Excellent time-management skills and a flexible, collaborative mindset
- Ability to work effectively within an interdisciplinary team and follow established care plans

- Treatment plan implementation with a focus on functional outcomes
- Patient-centered care and strong interpersonal communication
- Regulatory compliance and understanding of Medicare guidelines
