Shift Type: Monday-Friday, 8-hour day shifts

Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Evaluate and treat patients with communication and swallowing disorders in a home health setting
Develop and implement individualized care plans to improve patient outcomes
Collaborate with interdisciplinary team members to coordinate comprehensive care
Educate patients and families on therapeutic techniques and strategies
Document patient progress and maintain accurate clinical records
Travel to patient homes across a large territory covering multiple counties

Qualifications

Master's degree in Speech-Language Pathology from an accredited program
Current Kentucky SLP license (must be in hand at time of submission)
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
Active BLS certification
Minimum 1 year of clinical experience
Prior home health experience required

Education

Master's degree in Speech-Language Pathology

Knowledge & Skills

Strong clinical assessment and diagnostic skills
Excellent communication and interpersonal abilities
Ability to work independently and manage a diverse caseload
Knowledge of home health regulations and documentation requirements
Proficiency in electronic health records and tablet use
Time management and organizational skills for travel and scheduling
