Roles & Responsibilities

Develop personalized care plans in collaboration with physicians, patients, and families
Coordinate patient care through interdisciplinary communication and case conferences
Provide ongoing assessment of patients' clinical, functional, and emotional status
Document evaluations, treatment goals, and objectives accurately
Educate patients and caregivers on self-care techniques, medication, and diet

Qualifications

Licensed Registered Nurse in Vermont and New Hampshire (or compact license)
Graduate of an accredited school of nursing
Minimum 2 years of Home Health experience
Current BLS/BCLS certification
Strong clinical assessment and communication skills

Education

Graduate of an accredited school of nursing

Knowledge & Skills

Case management and patient assessment
Care plan development and interdisciplinary communication
Home health nursing and community health principles
Documentation and patient education
Proficiency with EMR systems like Point Click
