Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes
Develop and implement individualized treatment plans for post-surgery, fall recovery, and chronic conditions
Collaborate with patients, families, and interdisciplinary teams to promote optimal outcomes
Document patient progress using HomeCare/HomeBase EMR system
Cover a 60-mile radius around Valley City, ensuring patients receive compassionate care

Qualifications

Active North Dakota Physical Therapy license required
Current BLS certification from the American Heart Association
Valid driver's license and auto insurance
Minimum 1 year of experience as a Physical Therapist, preferably in home health
No clinical involvement or manager interview needed - streamlined process

Knowledge & Skills

Expertise in physical therapy evaluation and treatment techniques
Strong familiarity with home health settings and patient populations
Competent in using HomeCare/HomeBase EMR system
Excellent communication and time management skills
Ability to work independently and travel within a 60-mile radius
