Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Conduct comprehensive OASIS assessments for home health patients
- Develop and implement individualized care plans for elderly patients with falls, fractures, and orthopedic conditions
- Provide skilled physical therapy interventions to restore function and independence
- Document patient progress thoroughly using tablet-based tracking system
- Collaborate with interdisciplinary team to ensure coordinated care across four counties
Qualifications
- Active Physical Therapist license in Indiana (or compact eligible)
- Experience with OASIS documentation and home health preferred
- Ability to start new evaluation and start-of-care cases confidently
- Valid driver's license and reliable vehicle for travel across rural and suburban areas
Knowledge & Skills
- Expertise in geriatric rehabilitation and fall prevention
- Proficiency in OASIS assessments and home health regulations
- Strong clinical judgment and time management skills
- Comfortable using technology for patient charting and scheduling
