Roles & Responsibilities
- Conduct physical therapy assessments and interventions for patients
- Provide SOC visits, especially for orthopedic patients
- Utilize EMR system for patient records
- Manage a caseload of approximately 5 visits per day
Qualifications
- 2 years of experience in Physical Therapy (will consider less with Home Health experience)
- BLS certification required
- Experience with Home Health and Physical Therapy
Education
- Degree in Physical Therapy
Knowledge & Skills
- Physical Therapy techniques and best practices
- Home Health care principles
- EMR system (HCHB - HomeCare HomeBase)
- Strong communication and interpersonal skills
