Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct comprehensive start of care, recertifications, and resumption of care visits
Provide skilled physical therapy services in patients' homes
Complete 5-6 patient visits per day (or 3 visits plus an admission)
Perform OASIS assessments and maintain accurate documentation
Collaborate with clinical coordinators and sister agency therapists for support
Educate patients and families on home exercise programs and safety

Qualifications

A current and active North Dakota Physical Therapy License is required
BLS certification from the American Heart Association
Valid Driver's License and current Auto Insurance
Preference given to candidates with Home Health experience and OASIS training
First-time travelers are welcome to apply

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Proficiency in OASIS documentation and home health regulations
Excellent time management and ability to work independently
Compassionate patient communication and education
