Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Conduct comprehensive psychosocial assessments for patients in need
Coordinate discharge planning and connect patients with community resources
Provide crisis intervention and supportive counseling to individuals and families
Collaborate with interdisciplinary teams to ensure holistic patient care
Facilitate 5150 evaluations and involuntary holds as needed
Advocate for patients' rights and access to mental health services

Qualifications

Valid California LCSW license in good standing
Current BLS certification (AHA preferred)
Active 5150 certification (or ability to obtain prior to start)
Prior experience with Dignity Health or CommonSpirit facilities strongly preferred
Minimum 1 year of clinical social work experience in a hospital or mental health setting

Knowledge & Skills

Strong knowledge of mental health assessment and crisis intervention
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced healthcare environment
Proficient in care coordination and discharge planning
Understanding of California's 5150 laws and involuntary hold procedures
