Physical Therapist - Home Health
Location: Red Bluff, California
Start Date: 06-16-2026
End Date: 09-15-2026
Contract Type: Travel Contract
Shift Type: Day shift, Monday-Friday (no weekend requirement)
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Conduct comprehensive patient assessments in home settings
- Develop and implement individualized treatment plans
- Provide skilled physical therapy interventions to improve mobility and function
- Educate patients and caregivers on home exercise programs
- Collaborate with interdisciplinary team for optimal patient outcomes
- Maintain accurate and timely documentation in HCHB EMR
Qualifications
- Current Physical Therapy license in California
- BLS certification required
- Home Health experience required (travelers with less than 2 years considered if experienced in home health)
- Ability to travel within service area (Tehama County)
- Comfortable using HCHB EMR system
- Valid driver's license and reliable transportation
Knowledge & Skills
- Knowledge of home health regulations and OASIS documentation
- Skills in orthopedic rehabilitation and geriatric care
- Experience with patients with comorbidities (CHF, COPD)
- Proficiency in HCHB HomeCare HomeBase
- Strong communication and interpersonal skills
- Ability to work independently and manage a daily caseload
