Location: Louisville, KY

Start Date: 06-29-2026

End Date: 09-28-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekends and holiday rotation

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Perform start of care, recertifications, resumption of care, and evaluations for home health patients
Provide skilled physical therapy interventions to patients with diagnoses including CHF, ortho conditions, and pneumonia
Collaborate with clinical managers and educators for support in the field
Document patient progress and adjust treatment plans as needed
Educate patients and families on home exercise programs and safety

Qualifications

Current Physical Therapy license in Kentucky
BLS certification required
Home health experience preferred, but new grads considered with strong support
Ability to work independently and manage a caseload of 6.5 points per day

Knowledge & Skills

Strong clinical assessment and evaluation skills
Excellent communication and patient education abilities
Proficient in using electronic health records and provided tablet/phone
Comfortable with rotating weekend and holiday schedules
