Roles & Responsibilities

• Provide compassionate, skilled nursing care to residents in a long-term care setting
• Administer medications and treatments safely and document per facility policy
• Conduct assessments and maintain accurate clinical documentation
• Participate in MDS-related tasks when applicable (MDS experience preferred)
• Collaborate with nurses, therapists and facility staff to deliver resident-centered care
• Promote a safe, comfortable, and respectful environment for residents

Qualifications

• Current, unrestricted Colorado LPN license (required)
• Experience in Long-Term Care (LTC) (required)
• MDS experience (preferred)
• Comfort working day shifts in a LTC facility and collaborating with an interdisciplinary team
• Ability to meet facility onboarding requirements (e.g., credentials verification)

Education

• Current, unrestricted Colorado LPN license (required)
• Completion of an accredited practical nursing program (preferred)

Knowledge & Skills

• Long-term care practices and resident-centered care
• MDS documentation knowledge (preferred)
• Medication administration and basic nursing procedures
• Clear clinical documentation and communication skills
