Roles & Responsibilities

• Provide one-on-one occupational therapy in patients' homes to support independence
• Collaborate with an interdisciplinary team to create and implement person-centered care plans
• Educate clients and caregivers to promote function, safety, and goal achievement
• Assess functional needs across a broad cross-section of home health patients in central Vermont
• Document care accurately and timely in the EMR to support continuity and quality

Qualifications

• Active Vermont Occupational Therapy licensure (required)
• Home Health experience (required)
• Experience with McKesson or Netsmart EMR systems preferred
• Ability to commit to the assignment without requested time off during the contract
• Strong communication and collaboration skills for working with interdisciplinary teams

Knowledge & Skills

• Occupational therapy evaluation and intervention in home health settings
• Patient and caregiver education to support daily living and safety
• Interdisciplinary team collaboration and care coordination
• Familiarity with McKesson and/or Netsmart EMR platforms (preferred)
