Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living to keep residents comfortable and dignified
• Assist with bathing, dressing, grooming, toileting and mobility while promoting independence
• Take and record vital signs accurately and report meaningful changes to the nursing team
• Document resident care and behaviors clearly and promptly in the charting system
• Support safe patient transfers and use proper body mechanics to protect residents and yourself
• Observe residents for changes in condition and communicate concerns to nurses and leadership
• Work collaboratively with the care team to implement care plans and improve resident outcomes

Qualifications

• Active CNA certification in the practicing state (required)
• Current BLS/CPR certification preferred
• Comfortable providing hands-on care in a long-term care or rehabilitation setting
• Strong communication skills and a team-oriented attitude
• Ability to pass facility-required background checks, drug screening, and health screenings

Education

• High school diploma or GED
• Completion of an approved CNA training program and state certification

Knowledge & Skills

• Knowledge of basic nursing care principles and infection control practices
• Ability to safely assist with transfers, ambulation and use of assistive devices
• Accurate vital signs measurement and basic clinical observation
• Clear documentation and charting skills
• Empathy, patience, and strong interpersonal skills when working with residents and families
• Time management and the ability to prioritize tasks in a busy clinical setting
