Roles & Responsibilities

• Provide compassionate direct patient care and bedside support as an LPN
• Administer medications and treatments safely and accurately per facility protocols
• Document patient assessments, interventions, and responses thoroughly in the medical record
• Collaborate with RNs, physicians, and interdisciplinary team members to coordinate care
• Monitor patients for changes in condition and escalate concerns promptly
• Support patient safety and infection control best practices on every shift

Qualifications

• Current and unrestricted LPN license (must be valid in Vermont or compact status if applicable)
• Required: PCC experience - candidates MUST have PCC experience
• Current BLS (Basic Life Support) certification preferred or ability to obtain quickly
• Ability to work 12-hour night shifts, including every other weekend and some holidays
• Pass background check and meet facility onboarding requirements
• Strong communication skills and ability to work collaboratively on a travel assignment

Education

• Completion of an accredited LPN program and active LPN license

Knowledge & Skills

• Proficient in medication administration and basic nursing procedures
• Comfortable with electronic health record documentation
• Knowledge of infection control standards and patient safety practices
• Good assessment skills and ability to recognize changes in patient condition
• Time management, prioritization, and teamwork in fast-paced clinical settings
• Patient-centered communication and family engagement skills
