Roles & Responsibilities

• Provide compassionate, skilled nursing care to residents in accordance with care plans
• Administer medications and treatments safely and document responses
• Assess patient condition and communicate changes to the interdisciplinary team
• Maintain accurate, timely clinical documentation and handoffs
• Support residents with activities of daily living while promoting independence
• Participate in care team meetings and contribute to quality improvement efforts

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of practice or eligible to obtain one
• Basic Life Support (BLS) certification required; ACLS or other certifications encouraged
• Prior LPN experience in long-term care, skilled nursing, or similar settings preferred
• Immunizations and health screenings up to facility policy
• Strong communication skills and the ability to work collaboratively on a night-focused team

Education

• Diploma or certificate from an accredited Practical Nursing program (LPN/LVN)

Knowledge & Skills

• Skilled in medication administration and documentation
• Ability to perform resident assessments and recognize changes in condition
• Familiarity with long-term care workflows and electronic health records
• Good clinical judgment, time management, and prioritization under night-shift conditions
• Compassionate bedside manner and strong teamwork skills
