Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate nursing care to patients in a skilled nursing facility
Administer medications and treatments as prescribed
Monitor patient vital signs and report changes to the healthcare team
Document patient care accurately and timely
Collaborate with interdisciplinary team to ensure optimal patient outcomes
Educate patients and families on health management and discharge planning

Qualifications

Active Licensed Practical Nurse (LPN) license in Delaware
Current CPR certification
Completion of COVID-19 vaccination or approved exemption required
Must have a valid state or federal ID and wear photo badge at all times

Education

Graduation from an accredited LPN program

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Proficient in electronic health records (EHR) documentation
Ability to work independently and as part of a team
Time management and organizational skills
