Roles & Responsibilities

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care
• Develop and implement individualized plans of care following regulatory and clinical standards
• Utilize standardized assessments and evidence-based practice to guide clinical interventions
• Administer manual techniques, therapeutic exercises, modalities, and supportive devices
• Assess patient progress, adjust treatments, and document outcomes in the electronic record
• Provide consultation and counseling to patients, families, and caregivers
• Collaborate with the healthcare team on patient progress and treatment plans
• Prepare comprehensive discharge summaries and accurately record treatment minutes
• Supervise physical therapist assistants, rehabilitation aides, and students

Qualifications

• Minimum 1 year of Physical Therapy experience (new grads considered if prior Level II experience)
• LTC or Rehab experience required
• Active Oklahoma State License required
• Strong clinical skills in manual techniques, ambulation, modalities, and wound care
• Excellent communication and documentation abilities
