Roles & Responsibilities
- Provide compassionate direct patient care and assist patients with activities of daily living
- Support safe transfers, ambulation and mobility assistance with attention to dignity
- Monitor, record and report vital signs and changes in patient condition
- Ensure clear, timely documentation in the patient record
- Collaborate with nurses and rehab staff to deliver coordinated care plans
- Maintain infection control standards and a clean, safe environment
- Communicate effectively with patients and families to support comfort and understanding
Qualifications
- Current CNA (Certified Nursing Assistant) certification — required
- Basic Life Support (BLS/CPR) certification preferred or required per facility policy
- Ability to pass background check and drug screen as part of onboarding
- Reliable local transportation — this is a local-only assignment
- Strong communication skills and a patient-centered attitude
- Previous experience in long-term care or rehabilitation settings is a plus
Education
- Completion of an approved CNA training program; high school diploma or equivalent preferred
Knowledge & Skills
- Comfort with patient hygiene, ADLs and mobility support
- Accurate vital sign collection and basic clinical observations
- Familiarity with documentation and basic electronic medical records
- Understanding of infection control and safety procedures
- Time management and teamwork in a fast-paced care environment
- Compassionate bedside manner and ability to follow care plans
