Roles & Responsibilities
- Provide compassionate bedside care to residents and prioritize patient comfort
- Perform thorough patient assessments and communicate changes to the care team
- Administer medications and treatments accurately and document promptly
- Support wound care, IV/line observations, and routine clinical procedures as directed
- Collaborate with nurses, therapists and physicians to create and update care plans
- Ensure accurate charting in the medical record and follow facility policies for safety and infection control
Qualifications
- Current, active Licensed Practical Nurse (LPN) credential
- Comfortable working 12-hour night shifts including weekends
- Strong communication skills and ability to work well in a team
- Previous LPN experience in long-term care, rehab or similar setting is a plus
- Current BLS/CPR certification preferred or willing to obtain
Education
- Graduate of an accredited LPN program or equivalent practical nursing diploma
Knowledge & Skills
- Patient assessment and basic physical care
- Medication administration and documentation best practices
- Familiarity with electronic health record (EHR) charting
- Infection control and safety protocols
- Time management for 12-hour shift efficiency
- Team collaboration and clear clinical communication
