Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a skilled nursing and rehab setting
• Assess patients, monitor vital signs, and report clinical changes promptly
• Administer medications and treatments accurately and document per policy
• Collaborate with interdisciplinary team members to develop and follow individualized care plans
• Support admissions, transfers, and discharges with clear communication and professionalism
• Assist with rehabilitation activities and support restorative nursing goals
• Maintain accurate clinical documentation in the facility EHR and ensure compliance with regulatory standards

Qualifications

• Valid New Mexico LPN or RN license (active and in good standing)
• Current BLS (Basic Life Support) certification required
• Clinical experience in long-term care, skilled nursing, or rehab preferred
• Strong communication skills and ability to work collaboratively with a care team
• Ability to pass facility-required background and health screenings

Education

• LPN: Completed LPN diploma or certificate program and state licensure
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) and state licensure

Knowledge & Skills

• Patient assessment and critical thinking for acute and chronic conditions
• Medication administration and basic pharmacology knowledge
• Wound care fundamentals and restorative nursing principles
• Familiarity with electronic health records and clinical documentation
• Infection control best practices and resident safety focus
• Effective communication with patients, families, and interdisciplinary teams
