Roles & Responsibilities

• Provide compassionate bedside care and daily assessments for assigned patients
• Administer medications safely and document medication administration accurately
• Perform wound care, dressing changes, and other nursing procedures as needed
• Maintain clear, timely charting in the patient record and communicate changes in condition
• Collaborate with RNs, physicians, therapists, and support staff to coordinate care
• Educate patients and families on care plans, discharge instructions, and follow-up
• Support quality improvement initiatives and follow facility policies and standards

Qualifications

• Current LPN (Licensed Practical Nurse) license — New Mexico preferred/local applicants required
• Recent clinical experience in a similar setting (skilled nursing/rehab or acute care preferred)
• Basic Life Support (BLS/CPR) certification or willingness to obtain
• Ability to pass background checks and meet facility onboarding requirements
• Local-only candidates (position is designated LOCAL ONLY)

Education

• Practical nursing diploma or certificate (LPN program) or equivalent

Knowledge & Skills

• Strong patient assessment and monitoring skills
• Safe medication administration and documentation practices
• Comfortable with basic nursing procedures and wound care
• Good communication and teamwork — you keep the care team in sync
• Time management and prioritization during busy shift periods
• Familiarity with electronic health records and charting (adaptable to facility system)
