Roles & Responsibilities
- Administer medications and treatments accurately and safely according to care plans
- Perform focused patient assessments and report changes to the interdisciplinary team
- Monitor and document vital signs, intake/output, and patient responses in the chart
- Provide compassionate bedside care and support resident dignity and comfort
- Collaborate with RNs, physicians, and support staff to coordinate patient care
- Maintain clear, timely documentation and follow facility policies and procedures
- Assist with admissions, discharges, and transfers to ensure smooth transitions
Qualifications
- Current Licensed Practical Nurse (LPN) credential in good standing
- Valid basic life support (BLS/CPR) certification preferred or willingness to obtain
- Comfortable working 12+ hour night shifts (6:00 PM - 6:30 AM) on scheduled days
- Excellent communication skills and a team-oriented mindset
- Prior long-term care or skilled nursing experience is a plus
Education
- Completion of an accredited LPN/LVN program; high school diploma or GED required
Knowledge & Skills
- Medication administration and documentation best practices
- Ability to perform focused nursing assessments and monitor changes
- Familiarity with electronic medical records (EMR) and accurate charting
- Strong infection control and patient safety awareness
- Effective communication with patients, families, and clinical teams
