Roles & Responsibilities
- Assist licensed physical therapists in delivering high-quality rehabilitation care to residents
- Implement individualized therapeutic exercise programs and gait training to improve mobility
- Monitor patient progress and document treatment notes clearly and accurately
- Collaborate with nursing and interdisciplinary teams to coordinate care plans and ensure safety
- Educate patients and families on home exercise programs, fall prevention and adaptive strategies
- Maintain equipment, ensure treatment areas are safe and infection control standards are followed
- Support outcome-driven care by tracking goals and communicating changes to the supervising therapist
Qualifications
- Licensed Physical Therapist Assistant (PTA) or eligible for state licensure
- Graduation from an accredited PTA program
- Current Basic Life Support (BLS) / CPR certification preferred or willing to obtain
- Comfort working in long-term care and skilled nursing environments
- Strong interpersonal skills and a patient-centered approach to care
Education
- Associate degree from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Knowledge of therapeutic exercise principles, gait training and mobilization techniques
- Skilled in patient assessment, documentation and goal-oriented treatment planning
- Ability to use common rehab equipment and assistive devices safely
- Excellent communication, teamwork and time management skills
- Familiarity with infection control and safety protocols in a clinical setting
