Roles & Responsibilities
- Provide compassionate direct patient care and assistance with activities of daily living (ADLs)
- Support rehabilitation activities under the direction of nursing and therapy staff
- Monitor patients closely and report any changes in condition to the nursing team
- Document care accurately and timely to maintain high-quality patient records
- Help maintain a safe, clean, and respectful environment for residents and visitors
- Collaborate with nurses, therapists, and support staff to deliver coordinated care
Qualifications
- Current Certified Nursing Assistant (CNA) certification; T-CNA and Travel CNA experience highly valued
- Comfortable working 13-hour day shifts on the scheduled days (Monday, Wednesday, Saturday)
- Willingness to complete required onboarding, background check, and facility-required immunizations
- Strong communication skills and a team-oriented attitude
- Previous experience in a rehab or long-term care setting is a plus
Education
- High school diploma or equivalent and completion of a state-approved CNA program
Knowledge & Skills
- Skilled in patient transfers, mobility assistance, and fall prevention techniques
- Knowledgeable about infection control, safe patient handling, and basic clinical observations
- Able to chart clearly and follow care plans and physician/therapist directions
- Strong interpersonal skills for working with patients, families, and the care team
- Time management and prioritization skills to handle a busy shift
