Roles & Responsibilities

• Provide compassionate direct patient care and assistance with activities of daily living (ADLs)
• Support rehabilitation activities under the direction of nursing and therapy staff
• Monitor patients closely and report any changes in condition to the nursing team
• Document care accurately and timely to maintain high-quality patient records
• Help maintain a safe, clean, and respectful environment for residents and visitors
• Collaborate with nurses, therapists, and support staff to deliver coordinated care

Qualifications

• Current Certified Nursing Assistant (CNA) certification; T-CNA and Travel CNA experience highly valued
• Comfortable working 13-hour day shifts on the scheduled days (Monday, Wednesday, Saturday)
• Willingness to complete required onboarding, background check, and facility-required immunizations
• Strong communication skills and a team-oriented attitude
• Previous experience in a rehab or long-term care setting is a plus

Education

• High school diploma or equivalent and completion of a state-approved CNA program

Knowledge & Skills

• Skilled in patient transfers, mobility assistance, and fall prevention techniques
• Knowledgeable about infection control, safe patient handling, and basic clinical observations
• Able to chart clearly and follow care plans and physician/therapist directions
• Strong interpersonal skills for working with patients, families, and the care team
• Time management and prioritization skills to handle a busy shift
