Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents, promoting dignity and comfort
- Administer medications and treatments safely and accurately according to policy
- Assess residents regularly and report condition changes to the interdisciplinary care team
- Document care thoroughly and timely in the electronic health record
- Collaborate with families and staff to create person-centered care plans
- Assist with admissions, discharges, and transfers to ensure smooth transitions
Qualifications
- Active LPN license in Vermont or eligible to obtain Vermont licensure
- Current BLS/CPR certification (or willingness to obtain prior to start)
- Experience in long-term care, skilled nursing, or a related setting preferred
- Ability to pass background checks and required health screenings
- Strong communication skills and a patient-centered approach
Education
- Graduate of an accredited Practical Nursing program
Knowledge & Skills
- Medication administration and basic clinical procedures
- Wound care fundamentals and ADL assistance
- Accurate charting and familiarity with electronic health records
- Effective teamwork, time management, and prioritization
- Patient assessment, infection control, and safety practices
