Roles & Responsibilities
- Assess residents' psychosocial needs and develop individualized care plans that improve quality of life
- Coordinate services with clinical staff, families, and community partners to ensure continuity of care
- Advocate for residents' rights and access to appropriate resources and supports
- Facilitate discharge planning and referrals to community-based services as needed
- Maintain timely, accurate documentation and case notes in accordance with facility and regulatory standards
- Participate in interdisciplinary team meetings to align care goals and problem-solve complex cases
- Provide crisis intervention and short-term counseling to residents and families
Qualifications
- ME Social Worker License required (must hold current, active Maine license)
- Current clinical license in good standing with no restrictions
- Excellent interpersonal and communication skills for working with residents, families, and care teams
- Comfortable with documentation and electronic health record systems
- Previous experience in long-term care, skilled nursing, or community-based social work is a plus
Education
- Bachelor's degree in Social Work (BSW) preferred; Master of Social Work (MSW) a plus
Knowledge & Skills
- Strong assessment and care-planning skills
- Case management and care coordination
- Familiarity with Maine community resources and discharge planning
- Crisis intervention and short-term counseling techniques
- Regulatory and documentation standards for long-term care settings
- Collaborative teamwork and effective interdisciplinary communication
